Required Supporting Documents

In addition to this application, please prepare and submit the following documents separately.

A written statement describing your Conversion (on separate paper).
A written statement regarding your Call to the Ministry (on separate paper).
Three (3) Reference Letters: one from your Senior Pastor, and two from Ordained Ministers.

Application Fees

Application Fee: $100.00 — due with this application (non-refundable)
Two-Year Fee: $180.00 — payable before the Ordination Service

Please use dr.lewis@rsota.ca to e-transfer the $100.00 application fee. Please note, this fee is non-refundable.

The $180.00 two-year fee is payable before the Ordination Service and may also be e-transferred using the same address.
